A real ESA letter or PSD letter is issued only by the evaluating licensed provider when clinically appropriate. It is individualized, signed and delivered securely. Do not submit a sample document to a landlord or housing provider.
Provider identification, recommendation and date.
An actual document may identify the licensed provider, confirm the relevant professional relationship and state the provider’s recommendation within the scope of the evaluation. Specific format and content depend on the provider and circumstances.
